REQUEST FORM

This form is used to request that a new or replacement radio be added to the MDICE system.  This form must be completed before the radio can be programmed or placed into service on the system.

Template:  
The template is the channel lineup.  Most agencies only have one or two, but we'll need to know which one to use in programming your radio.  If unsure, please contact MDICE managers.
Radio Alias:  
The Radio Alias is a unique identifier for that radio,  this is how the radio displays on the METCAD console in the event of an emergency button activation (if so equipped), 8 character maximum.
Display Radio Alias:  
This alias is what displays on the LCD screen on the radio (if so equippoed), it can be the officers badge number to whom the radio is assigned, a vehicle number or inventory number, 8 character maximum.
User/Function Information:  
This information is only used to help identify the radio in the master listing.  It could be the officer name, badge number, id number, function name, etc.  Examples: CPD7745; Engine 252; Chief 600
